Background check

Healthcare Sanctions Screening

Check named healthcare exclusion or sanctions sources, or discuss an available contracted screening product.

A healthcare administrator reviews provider records at a clinic desk.
Service formatSelected exclusion and sanctions searches

Know which healthcare sources are checked.

Choose the exclusion or disciplinary sources your organization needs reviewed. We’ll name the sources or contracted product and explain its coverage. FACIS Level 1 and Level 3 are distinct options and depend on provider availability.

Results identify the review date and any record-matching or clarification steps. A product name does not determine agency eligibility, and a one-time search does not include continuous monitoring.

What’s included.

Identify the subject and screening purpose

Identify the individual or entity and the business purpose before selecting sources.

Confirm the product and coverage

Name the contracted product and coverage in the order. If FACIS Level 1 or Level 3 is requested, confirm the specific product, current provider terms and sources covered rather than assuming the two options return the same information.

Distinguish LEIE from broader products

HHS-OIG maintains the LEIE for currently excluded individuals and entities. A defined LEIE search is different from a broader product covering additional sanctions sources.

Date the search and review matches

State the review date and any matching or clarification steps. Continuing monitoring and eligibility decisions are outside a one-time search unless separately agreed.

What you’ll receive

  • Named sources or healthcare product scope.
  • Available search results and review date.
  • Identified matches requiring clarification.
  • Completion status and coverage limitations.

How it works.

  1. Name the sources or product

Confirm the sources or contracted product appropriate to the request.

  2. Check the authorized identifiers

Use the authorized identifiers and matching workflow.

  3. Review matches and questions

Separate source results, unresolved matches, and any further agency or organizational decision.

Know what to expect.

We’ll confirm the work, the updates you’ll receive and who to contact along the way.

  • The named source list or contracted product, coverage and one-time or separately agreed recurring scope.
  • The identifiers used and any agreed match-resolution step.
  • How possible matches, completed source results and coverage gaps will be reported to the healthcare reviewer.

What helps us help you.

  • Person or entity type and intended screening purpose.
  • Required source list or client screening specification.
  • Accurate authorized identifiers.
  • One-time volume and any separately proposed recurring requirement.

What shapes your quote.

  • Sources or provider product selected.
  • Individual versus entity requests.
  • Matching and clarification scope.
  • Volume and any separately contracted recurring searches.

Tell us what you need help with. We’ll explain which records are needed and how to provide them. Personal reports, ID images and account files can wait until then.

Frequently asked questions

Questions about this service?

A few details to help you decide.

Ask another question
Is FACIS Level 1 the same as a Florida Level 1 background check?

No. FACIS Level 1 is a healthcare sanctions product label. It is different from Florida’s statutory screening terminology; ask which product and sources are actually included in the proposed order.

Does this replace a Clearinghouse eligibility determination?

No. A sanctions search is a defined screening component. Agency screening and eligibility processes follow their own requirements and authorized route.

Is monthly monitoring included?

Not by default. The order must distinguish a single search from any separately available recurring product.

Does every healthcare package contain the same sources?

No. Require the source list or provider coverage definition so the package matches the requested screening purpose.
