Fingerprint Cards / FD-258

Explore the fingerprint card your application requires. Availability, extra copies and any submission work need confirmation.

Service formatPer card or additional copy

Get the card format right before your appointment.

Some applications require a physical fingerprint card, such as an FD-258. Share the receiving organization’s instructions, card format and copy count so we can check availability before arranging an appointment.

Extra cards may be added to an accepted order. Mailing, agency processing and electronic conversion are separate unless included; conversion also requires an approved provider route. A completed card is one part of the application, not a completed background screening.

What’s included.

Confirm the required card format

Specify the exact card and receiving organization; do not assume one format is accepted for every purpose.

Specify the card and extra copies

The request covers the completed card you specify. Extra copies from the same visit can be added to the order.

Separate capture from submission charges

Agency submission, mailing, and processing charges are separate unless included in the order.

Check the card-conversion pathway

Conversion of an existing card to electronic submission is optional and depends on an approved provider pathway.

What you’ll receive

  • Confirmed card type and quantity.
  • Completed cards within the accepted capture scope.
  • Any agreed delivery or handling instructions.
  • Clear identification of submission work that remains separate.

How it works.

  1. Confirm the card and copies

Review the receiving organization’s instructions and quantity.

  2. Prepare cards at the available appointment

Prepare the agreed cards during the available appointment.

  3. Receive the cards or agreed handling

Return the cards or complete any separately accepted handling step.

Know what to expect.

  • The card type, number of cards and receiving organization’s original instructions.
  • Whether the accepted work ends at capture or includes specific mailing, delivery or conversion.
  • The handoff of completed cards and the arrangements for a format or print-quality question.

What helps us help you.

  • Optional mailing or delivery.
  • Separately accepted conversion or agency submission.

Tell us what you need help with. We’ll explain which records are needed and how to provide them. Personal reports, ID images and account files can wait until then.
